Try running top (or htop) to figure out where your memory is going, the swap usage is just a side affect of the excessive memory usage.

My server was running on Plesk 8.6 (Centos 5.0) i have upgraded to Plesk 9.0 and PHP 5.2.8
Since when upgrade i have a problem on my servers.
And i need to reboot everyday. Everyday memory usage reaches to %50 - %60 and then swap usage reaches to 100%.
How can i fix it ?
And how can i upgrade kernel version by using ssh ?
